Desmond finds himself living with his ancestors and their friends--Altair, Ezio, Malik, Connor, Leo, etc.--and life is good; however, Desmond finds himself looking for Shaun, who is strangely not around and cannot be found. In the end he realize that this is his mind, after he had been...electrified?...by Juno, and his body is either dead or a vegetable; Shaun's not here because Shaun's not dead. Or, Desmond doesn't realize and we, the readers, realize it.
